Fill out our membership application

Step 3)

Come back in 2 days.

Our two day waiting period can be waved for SEVERE diseases/medical conditions such as: Cancer, AIDS, Multiple Sclerosis, Parkinson’s, Seizure disorders, PTSD, Tourette’s, Glaucoma, Rheumatoid Arthritis, and other severe pain conditions.

To avoid the 2 day waiting period

Two days before your first time to the SJCBC, FAX your Doctors Recommendation and California ID to: 408-247-0436
